(outdated) Instruction on how to fill in Z-15 form

Probably you live and work in Poland and  it happens you need to take care after you sick child or sick relative.

I can imagine how difficult it can be fo r you to fill in Z-15 form. It is very difficult even for Poles:)

I decided to help you with that by translating all sections of this form into english.
